THE MISSION
Nolla is building an ecosystem of care for everyone. AI-driven healthcare that gives every person access to the kind of medicine that used to require a doctor in the family. We make this real through a consumer app that handles diagnosis, prescription, and treatment tracking end to end, with general primary care launching this summer. We're at the forefront of medical AI research and targeting peer-reviewed clinical publication. We raised a $4.5M seed from General Catalyst.
THE ROLE
We're looking for a founding engineer to own product and AI systems end to end. The work blends consumer product engineering with AI infrastructure: shipping patient-facing features, building the clinical AI layer that powers diagnosis and treatment selection, and integrating the health systems that make care delivery real. You'll work directly with the founding team and your work reaches patients immediately.
WHAT YOU'LL BUILD
Consumer Product
- Ship features across iOS and web that patients interact with daily: intake flows, scan mechanics, treatment tracking, prescription management, and messaging
- Own frontend and backend systems end to end, from design to production
Clinical AI Systems
- Build and maintain the LLM inference layer powering clinical interview, differential reasoning, and treatment selection
- Implement agentic workflows, tool-calling, and evaluation loops that make AI reliable in a regulated environment
- Develop abstention, routing, and fallback strategies that favor safety and correctness
Health Data Infrastructure
- Build HIPAA-compliant pipelines connecting patient data, EHR integrations, pharmacy systems, and telehealth partners
- Unify fragmented health systems so every patient interaction is accurate, traceable, and explainable
- Develop internal tooling that clinical ops and physicians use daily
Platform & Scale
- Architect systems that support rapid product expansion: new verticals, new states, new clinical modalities
- Own reliability, security, and performance across a healthcare platform with real patients
WHAT YOU BRING
- 3+ years of full stack engineering experience, with meaningful time at an early-stage startup
- Strong TypeScript and Python across frontend and backend
- Experience building with LLMs in production: agents, tool-calling, prompt engineering, eval frameworks
- Comfort working across the stack, from schema design to production debugging
- Comfort in HIPAA-regulated environments or willingness to get up to speed fast
Bonus Points
- iOS or mobile engineering experience
- Familiarity with FHIR, HL7, or healthcare data standards
- Experience with observability stacks: Langfuse, Sentry, or similar
- Prior work in clinical AI, digital health, or other regulated domains

ROLE LOGISTICS, COMPENSATION & BENEFITS

- Salary: $140,000‑$180,000, based on experience
