About the role

Position Summary

Bay Area Rehabilitation Center (BARC) is looking for a compassionate and experienced Program Manager to lead our HCS and TxHmL programs. In this role, you’ll support a dedicated team, guide high-quality service delivery, and help ensure individuals and families receive the care and support they need to thrive.

This position is ideal for someone who enjoys both leadership and hands-on involvement in case management, while making a meaningful impact in the lives of under served populations.

What You’ll Do

  • Oversee daily operations of HCS and TxHmL programs, ensuring services are high-quality and compliant
  • Lead and support case managers, ISS staff, and billing coordination
  • Guide the development and monitoring of individualized care plans and Person-Directed Plans (PDPs)
  • Ensure clients receive consistent follow-up, advocacy, and coordinated care
  • Monitor program performance, service utilization, and documentation standards
  • Conduct internal audits and help prepare for HHSC reviews and surveys
  • Partner with the billing team to ensure accurate, timely billing and authorizations
  • Serve as a key point of contact for families, staff, and external partners
  • Foster a positive, collaborative team environment focused on dignity, respect, and equity.

Requirements

What We’re Looking For

  • Bachelor’s degree in Social Work, Human Services, or related field (Master’s preferred)
  • 3+ years of experience in IDD services, with at least 2 years in HCS/TxHmL
  • Knowledge of/experience with Health and Human Services Waiver Program
  • Previous leadership or supervisory experience
  • Strong knowledge of TAC and HHSC requirements
  • Experience with case management, care coordination, and compliance
  • Excellent communication, organization, and problem-solving skills
  • Comfort with documentation systems and basic technology
  • Bilingual skills are a plus

Additional Requirements

  • CPR/First Aid certification
  • Valid driver’s license and reliable transportation
  • Ability to travel locally to homes and program sites as needed
